HealthMed Diagnostic & Treatment Center is a rapidly expanding multispecialty Article 28 healthcare facility committed to providing comprehensive, patient-centered care to our diverse community. We are seeking a compassionate, motivated, and experienced Licensed Clinical Social Worker to help build and grow our behavioral health department.
This is an exciting opportunity to work alongside primary care providers, psychiatrists, psychologists, and specialists in a collaborative, integrated healthcare setting.
Responsibilities
- Conduct comprehensive psychosocial and behavioral health assessments.
- Provide individual, family, and crisis counseling.
- Develop and implement individualized treatment plans.
- Perform diagnostic evaluations and maintain accurate clinical documentation.
- Coordinate care with primary care physicians, psychiatrists, psychologists, and community resources.
- Assist patients with care coordination, referrals, and accessing community services.
- Maintain compliance with all federal, state, and organizational regulations.
- Participate in multidisciplinary case conferences and quality improvement initiatives.
Qualifications
- Active New York State LCSW license required.
- Master's Degree in Social Work (MSW) from an accredited institution.
- Minimum 1 year of clinical experience preferred (new graduates with strong clinical training are encouraged to apply).
- Experience working with children, adolescents, and adults is a plus.
- Excellent communication and interpersonal skills.
- Ability to work effectively within a multidisciplinary healthcare team.
- Bilingual candidates are highly encouraged to apply. Bengali, Punjabi, Hindi, Spanish, and Urdu speakers are especially welcome.

Schedule
Our behavioral health program currently operates:
- Monday–Friday: 5:00 PM – 9:00 PM
- Saturday: 9:00 AM – 5:00 PM
Additional daytime hours may become available as the program expands.
