We're seeking an experienced Registered Nurse (RN) Case Manager to provide clinical oversight and support to adults receiving services in our 245D community-based programs and two six-bed 144G assisted living homes in the Roseville area.
This position offers significant flexibility and autonomy while allowing you to make a meaningful impact across a small group of residential settings.
Position Summary
The RN Case Manager serves as the primary clinical resource for assigned homes, ensuring residents receive high-quality, person-centered care while maintaining compliance with Minnesota 245D and 144G regulations. Working closely with program leadership, direct support professionals, and providers, you'll oversee assessments, nursing services, care planning, medication management, and clinical education.

What You'll Do
Provide nursing oversight for assigned 245D and 144G residential programs.
Complete initial and ongoing nursing assessments.
Develop, implement, and update individualized service and care plans.
Perform resident admissions and nursing evaluations.
Provide medication management oversight and physician order implementation.
Complete skilled nursing visits as needed, including wound care, catheter management, diabetic support, and health monitoring.
Respond to changes in resident condition and collaborate with healthcare providers.
Train and mentor direct support professionals and caregivers on delegated nursing tasks.
Participate in quality assurance initiatives and regulatory compliance activities.
Maintain accurate clinical documentation and ensure compliance with state regulations.
Serve as a collaborative resource for managers, staff, residents, and families.
Qualifications
Current Minnesota Registered Nurse (RN) license in good standing.
Minimum of two years of nursing experience preferred.
Experience in assisted living, group homes, home care, developmental disabilities, or long-term care strongly preferred.
Knowledge of Minnesota 245D and/or 144G regulations is preferred, but training will be provided to the right candidate.
Strong assessment, clinical decision-making, and organizational skills.
Excellent communication and relationship-building abilities.
Valid driver's license and reliable transportation.
