Responsibilities
- Home health evaluations, treatments, and discharge planning
- Develop individualized care plans
- Complete OASIS documentation
- Coordinate care with physicians and interdisciplinary teams
- Provide orthopedic, neurological, and geriatric rehab services
Requirements
- Active Physical Therapist license (CO or eligible)
- Strong clinical and communication skills
- Home health experience preferred (not required)
