Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
- Monitor and observe patients’ health status by taking vital signs, recording medical documentation accurately, and reporting any changes promptly.
- Administer medications following prescribed schedules while maintaining strict adherence to home health care regulations and HIPAA confidentiality standards.
- Support clients with meal preparation and nutrition needs, ensuring dietary guidelines are followed for optimal health.
- Provide companionship and emotional support to clients, especially those with dementia or Alzheimer’s disease, fostering a positive and engaging environment.
- Maintain a clean and safe living environment by assisting with light housekeeping tasks such as laundry, cleaning, and organizing personal spaces.
- Document all patient interactions and medical records using Electronic Health Record (EHR) systems or EMR systems efficiently and accurately.
- Follow protocols for hospice or end-of-life care when applicable, respecting the client’s wishes and dignity at all times.
- Assist with medical procedures like catheter care or other specialized tasks as trained, ensuring compliance with safety standards.
- Support clients during transfers or mobility exercises using proper patient transfer methods to prevent injury.
Experience
- Prior experience working in assisted living facilities, nursing homes, or home healthcare agencies is highly preferred.
- Demonstrated experience caring for geriatric patients or individuals with disabilities such as developmental disabilities or dementia is advantageous.
- Knowledge of medical terminology, medication administration procedures, and vital signs measurement is essential.
- Experience working with EMR/EHR systems for documentation purposes is desirable.
- Certification in CPR and First Aid is required; additional training in hospice care or heavy lifting techniques is a plus.
- Familiarity with home health care regulations and maintaining patient confidentiality under HIPAA guidelines is necessary.
- Proven ability to work independently while communicating effectively with clients and team members in a compassionate manner.

Pay: $17.50 - $19.00 per hour
