Identifies, prevents, and resolves medication-related problems, adverse drug events, drug interactions, therapeutic duplications, and medication errors.
Performs pharmacokinetic dosing, therapeutic drug monitoring, medication adjustments, and pharmacist consults in accordance with established protocols.
Compounds, prepares, dispenses, verifies, and documents medications in accordance with organizational policies, regulatory requirements, and professional standards.
Participates in interdisciplinary rounds and collaborates with physicians, nurses, case managers, and other healthcare professionals to develop and optimize medication treatment plans.
Provides medication education and counseling to patients, families, caregivers, and healthcare team members regarding safe and effective medication use.
Supports transitions of care initiatives from admission through discharge and follow-up to ensure continuity of medication management.
Participates in antimicrobial stewardship, medication surveillance, formulary management, and restricted medication programs.
Serves as a drug information resource for patients, providers, and healthcare team members.
Supervises, mentors, trains, and evaluates pharmacy technicians, pharmacy interns, students, residents, and other pharmacy personnel.
Utilizes electronic health records, pharmacy information systems, clinical decision support tools, and medication automation technologies to promote safe and efficient medication use.
Identifies, documents, reports, and participates in the resolution of medication safety events, adverse drug reactions, and medication variances.
Participates in quality improvement initiatives, performance improvement activities, regulatory compliance efforts, and accreditation readiness activities.
Monitors and tracks clinical interventions, medication safety outcomes, quality metrics, cost-saving initiatives, and departmental performance indicators.
Contributes to professional advancement through committee participation, research activities, specialty practice development, continuing education, and scholarly activities.
Assists with daily pharmacy operations to ensure effective workflow and efficient patient care services.
Performs other duties as assigned.
