Job Overview
We are seeking a compassionate and motivated Registered Nurse to lead wound care treatments and follow up in our facility. This role includes assessing and treating wounds and skin conditions, rounding with the physician, ensuring accurate documentation, and educating nursing staff on proper wound care practices. You will be overseeing the care and treatment of residents both in our Long-Term Care units and Short-Term Rehabilitation units. This a position for an RN who is detail oriented and organized.
Responsibilities
- Conduct thorough patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' vital signs and report any changes in condition to the healthcare team.
- Provide education and support to patients and their families regarding health management and treatment options.
- Collaborate with interdisciplinary teams to coordinate care for patients, including those requiring infectious disease care or ostomy care.
- Utilize electronic health record systems for documentation and patient management.
- Orient new employees to the importance of wound care policies and procedures, personal hygiene, hand washing techniques, etc., as well as the employee's responsibility in following our established procedures for tasks involving exposure to blood/body fluids.
Skills
- Proficient in patient assessment techniques and clinical decision-making.
- Knowledge of coding practices relevant to nursing documentation.
- Familiarity with wound care best practices and documentation.
- Familiarity with case management principles and practices.
- Strong communication skills, both verbal and written, for effective patient interaction.
- Ability to work collaboratively within a multidisciplinary team environment.
- Detail-oriented with strong organizational skills to manage multiple patient cases effectively.
- Wound care education and/or certification is preferred.
- Knowledge and experience using Point Click Care (PCC) preferred.
