The Licensed Social Worker is responsible for supporting the emotional, behavioral, educational, and social well-being of children residing in our residential programs. Reporting to the Director of Social Work, this position develops comprehensive psychosocial assessments, collaborates with interdisciplinary teams, and advocates for each child's individual needs. The successful candidate will demonstrate professionalism, compassion, and a commitment to the client's mission, core values, policies, and all applicable federal, state, and local regulations.
Key Responsibilities:
This position requires full understanding and active participation in fulfilling the mission of the client. It is expected that the employee demonstrates behavior consistent with core values as well as follows all legal requirements in implementing duties according to policies and procedures. The Social Worker reports to the Director of Social Work and is responsible for collaboratively developing a Psycho-Social history for each child. This history will include detailed information about the child's previous family background, medical history, placement history prior to their arrival at the client's, services provided during their time with Second Family, Inc., and future plans tailored to their individual needs.
- Conduct comprehensive psychosocial assessments for each child.
- Develop and maintain detailed psychosocial histories, including:
- Family and social background
- Medical and behavioral history
- Placement history
- Services received while at the client
- Discharge planning and future care recommendations
- Develop, implement, and monitor Individual Service Plans (ISPs).
- Participate in the development and review of Individualized Education Programs (IEPs).
- Develop and monitor behavioral support plans for children receiving psychotropic medications or requiring behavioral interventions.
- Provide case management and advocacy services for assigned children.
- Coordinate with physicians, nurses, educators, therapists, guardians, and community agencies to ensure continuity of care.
- Attend and participate in interdisciplinary treatment meetings.
- Represent the client during:
- School meetings
- Home visits
- Court hearings and reviews
- Community appointments
- Maintain accurate, timely, and confidential documentation in accordance with agency standards and regulatory requirements.
- Travel between the client's residential locations as needed.
Required Qualifications:
- Master's Degree in Social Work (MSW) from an accredited School of Social Work.
- Current Maryland Social Work license (LMSW, LCSW, or LCSW-C as applicable).
- Minimum of one (1) year of supervised social work experience in a healthcare, residential, behavioral health, or related setting working directly with individuals.
- Current American Heart Association CPR and First Aid certification (or ability to obtain upon hire).
- Valid driver's license with reliable transportation and the ability to travel between program locations.
