Paradigm Rehabilitation is seeking an experienced and motivated In-Homecare Physical Therapy Supervisor to support our growing home care therapy team throughout New York City’s five boroughs. This part-time leadership role is ideal for a licensed Physical Therapist who enjoys clinical oversight and ensuring the highest quality of patient care.
Position Summary
The Physical Therapy Supervisor will be responsible for completing initial evaluations and reassessments, providing clinical supervision to a team of four per diem Physical Therapist Assistants (PTAs), reviewing and co-signing documentation, and ensuring compliance with all regulatory and agency requirements. This role combines direct patient care with clinical leadership and offers the opportunity to make a meaningful impact on both patient outcomes and clinician development.
Because the PTAs are per diem clinicians, schedules and caseloads may vary based on patient referrals and agency needs. The Physical Therapy Supervisor must be flexible, responsive, and available to provide ongoing clinical oversight, documentation review, and support to their assigned team.
Responsibilities
- Complete Physical Therapy evaluations, reassessments, and supervisory visits throughout the five boroughs of New York City.
- Provide clinical supervision and support for a team of four per diem Physical Therapist Assistants (PTAs).
- Review, approve, and co-sign PTA treatment notes and documentation in accordance with New York State regulations.
- Ensure all documentation is completed accurately, timely, and in compliance with agency, payer, and regulatory requirements.
- Monitor patient progress and modify treatment plans as needed.
- Participate in case conferences, team meetings, and quality assurance initiatives.
- Maintain productivity standards while delivering exceptional patient care.
- Assist with managing and supporting PTA caseloads, which may fluctuate based on referral volume, geographic coverage, and patient census.
- Serve as a clinical resource for PTAs and provide timely guidance regarding patient care and documentation.
Time Commitment & Availability
- Part-time position with borough-wide coverage throughout New York City.
- Must maintain flexibility to accommodate varying PTA schedules and patient needs.
- Expected to be available on-call hours (These hours vary as the PTAs have differing schedules- Hours per week will average to 20-24)
- Ability to prioritize and respond to time-sensitive clinical matters as they arise.
- Travel throughout the five boroughs is required- Travel Reimbursement is provided in addition to the salary.
Qualifications
- Graduate of an accredited Physical Therapy program.
- Minimum of 2 years of clinical Physical Therapy experience required.
- Previous supervisory and/or home care experience strongly preferred.
- Strong knowledge of New York State Physical Therapy practice regulations and documentation requirements.
- Excellent communication, organizational, and leadership skills.
- Ability to work independently while managing multiple clinicians and patient needs across a large geographic area.
