Overview
The Case Worker at SMSS Youth Group Home is responsible for managing the individual case files of all male youth residents (ages 13–17) and coordinating the full continuum of services required to keep each resident safe, healthy, and actively engaged in the program. The Case Worker serves as the primary point of contact for each resident's service needs — scheduling and tracking medical, behavioral health, educational, and community appointments; maintaining accurate and up-to-date case records; and ensuring each young man is empowered with the tools, supports, and opportunities he needs to thrive during his placement and beyond. This position reports directly to the Program Director.
Duties
- Maintain accurate, complete, and organized case files for each resident in accordance with 55 Pa. Code § 3800.20 and SMSS policy; ensure all required documentation is on file prior to or at admission
- Coordinate and track the completion of all required intake documents including admission agreements, consents, release forms, resident rights acknowledgments, and medical authorizations
- Ensure each resident's Individualized Service Plan (ISP) is current, reviewed at required intervals, and accurately reflects the resident's needs, goals, and progress
- Collaborate with the Youth Development Coach and direct care staff to ensure residents have access to mentorship sessions, life skills instruction, workforce development activities, entrepreneurship education, health education, and recreational opportunities consistent with the SMSS program model
- Support residents in setting and working toward personal goals related to education, employment readiness, health, and community integration Facilitate or co-facilitate resident group discussions, community meetings, or individual check-ins as needed to promote engagement, self-advocacy, and emotional well-being

Requirements
- Proven experience working with children across various age groups such as infants, toddlers, preschoolers, or students with disabilities
- Strong classroom management skills with the ability to lead a team effectively
- Knowledge of childhood development principles and early childhood education practices
- Certification in first aid and CPR is required; additional certifications in special education or working with individuals with disabilities are highly desirable
- Prior experience in daycare settings or nannying/babysitting roles demonstrating caregiving expertise
- Excellent communication skills to collaborate with staff, parents, and community partners
- Ability to handle challenging behaviors with patience and professionalism
