We are seeking a compassionate, organized, and motivated Case Manager to join our behavioral health team. In this vital role, you will serve as an advocate and guide for clients receiving mental health and substance use treatment. You will coordinate services, develop individualized treatment plans, connect clients with community resources, and collaborate closely with therapists, medical providers, and interdisciplinary staff to support each client's recovery journey.

Responsibilities
- Conduct comprehensive biopsychosocial assessments and assist in developing individualized treatment plans in collaboration with the clinical team.
- Coordinate client care by maintaining regular communication with therapists, medical providers, referral sources, family members (when authorized), and community agencies.
- Monitor client progress toward treatment goals and document services accurately, timely, and in accordance with regulatory and organizational standards.
- Assist clients in accessing community resources including housing, employment, education, transportation, financial assistance, legal services, medical care, and other supportive services.
- Facilitate client orientation, treatment planning sessions, discharge planning, and continuity of care to promote successful long-term recovery.
- Provide crisis intervention, problem-solving support, and advocacy while helping clients identify and overcome barriers to treatment participation.
- Maintain accurate electronic health records (EHR), complete required documentation, and ensure compliance with DHCS, HIPAA, and accreditation standards.
- Collaborate with an interdisciplinary treatment team including therapists, counselors, nurses, physicians, and support staff to ensure coordinated, client-centered care.
- Participate in case consultations, treatment team meetings, supervision, and ongoing professional development opportunities.
- Promote client engagement through motivational interviewing, strengths-based practices, and trauma-informed care principles.
- Maintain professional boundaries while fostering a safe, respectful, and supportive therapeutic environment.
- Perform other duties as assigned to support client care and organizational operations.
Qualifications
- Any of the following credentials: RADT, CADC (I–IV), CATC, AMFT, APCC, ASW, LMFT, LCSW, LPCC, or comparable qualifications
- Experience working with individuals experiencing mental health disorders, substance use disorders, or co-occurring conditions preferred.
- Knowledge of behavioral health treatment principles, recovery-oriented care, trauma-informed practices, and case management services.
- Familiarity with DHCS regulations, Medi-Cal documentation standards, HIPAA requirements, and behavioral health compliance preferred.
- Strong organizational skills with the ability to manage multiple client cases while meeting documentation deadlines.
- Excellent verbal and written communication skills with the ability to build rapport with clients, families, providers, and community partners.
- Proficiency with Electronic Health Record (EHR) systems and Microsoft Office applications.
- Ability to work independently while contributing effectively within a multidisciplinary treatment team.
