About the Role
The Health Educator will empower individuals living with HIV to achieve better health outcomes through education, advocacy, and personalized support. Working with underserved and marginalized communities, this position assists clients navigating healthcare services, promotes medical and medication adherence, accompanies them to appointments, facilitates support groups, and connects them with essential medical, behavioral health, and community resources.
Key Responsibilities
· Conduct client assessments to identify barriers to HIV treatment adherence and develop individualized care plans that promote positive health outcomes.
· Provide one-on-one education, support, and evidence-based group programming focused on HIV care, medication adherence, and healthy living.
· Coordinate care by connecting clients with medical, behavioral health, case management, and community resources while collaborating with providers and multidisciplinary teams.
· Monitor client progress, track health outcomes, maintain accurate documentation, and adjust service plans to support ongoing engagement in care.
· Advocate for clients, promote independence through healthcare navigation and education, and conduct outreach and client visits in community, medical, and home-based settings.
Qualifications
- Bachelor's degree in human services or related field OR comparable years of qualifying experience.
- Experience in education and HIV prevention activities preferred.
- Minimum one year of experience within a social service setting or a related field.
- Demonstrated ability to collaborate with colleagues and deliver respectful, high-quality care to patients in a community health setting across wide range of backgrounds and perspectives.
- Proficiency in Microsoft Office with strong attention to detail.
- Excellent communication, organizational, and problem-solving skills.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Valid Florida driver's license, proof of insurance, and reliable transportation.
- Must pass Level II background check, fingerprinting, and E-Verify employment eligibility.
