MDS Coordinator

Responsibilities:
Coordinate the collection, validation, and submission of Minimum Data Set (MDS) assessments, care plans, and resident information in a timely and accurate manner
Collaborate with healthcare providers, therapists, and other team members to ensure comprehensive care plans that meet each resident's unique needs
Review and analyze MDS data to identify trends, patterns, and areas for improvement, and implement changes as needed
Develop and maintain effective relationships with residents, families, and healthcare providers to ensure excellent customer service and communication
Participate in quality improvement initiatives and regulatory compliance efforts to maintain our high standard of care
Maintain accurate and detailed records of all MDS assessments, care plans, and resident information
