GLC Group is currently seeking a Licensed Practical Nurse (LPN) for PRN/Per Diem shifts in Milwaukie, OR.

Compensation:
- Hourly: $39
- Weekly Pay - every Friday
Job Requirements:
- Valid and current LPN License in the state you are applying to
- 1-2 years LPN current/recent experience in Assisted Living/Memory Care
- Active Certifications - BLS
- Must have a COVID-19 vaccination or exemption where applicable.
Key Responsibilities:
- Administer daily oral, topical, and injectable medications to residents while accurately maintaining electronic health records and monitoring for adverse side effects.
- Conduct routine physical health assessments, take vital signs, and monitor residents for subtle changes in cognitive, emotional, or physical condition, updating individual care plans accordingly.
- Implement tailored, person-centered care strategies for residents with Alzheimer's, dementia, or cognitive decline, utilizing gentle redirection, de-escalation, and structured daily routines.
- Direct and support Certified Nursing Assistants (CNAs) and caregivers in delivering daily personal care activities, ensuring high standards of resident dignity, safety, and hygiene.
- Collaborate with Registered Nurses, attending physicians, and families to report health updates, process physician orders, and assist with resident admissions or transfers.
