- diverse caseload of Medicaid recipients, helping them access vital healthcare services, coordinate medical appointments, and connect

Key Responsibilities:
- . Conduct assessments to identify health and social service needs
- . Develop personalized care plans and coordinate with medical providers
- . Assist clients in navigating the healthcare system and accessing resources
- . Monitor client progress and provide ongoing support
- . Advocate for clients and address barriers to care
Qualifications:
- . College degree (preferably in Social Work, Public Health, or related field)
- . Strong communication and organizational skills
- . Ability to work effectively with diverse populations
- . Bilingual in Spanish is a plus

REQUIREMENTS:
- Must be fully vaccinated against covid-19
- Fluency in both English and Spanish.
- Bachelor’s Degree Master A plus
- Two (2) years' experiences in successful community outreach/recruitment or volunteer coordination preferred.
