Responsibilities:
Provide direct patient care, including administering medications, wound care, and other treatments as ordered by physicians and healthcare providers
Establish and maintain positive relationships with residents, families, and healthcare team members
Conduct routine assessments, monitor vital signs, and record medical information accurately
Communicate effectively with physicians, nurses, and other healthcare professionals to ensure seamless care coordination
Implement infection control measures and maintain a clean and safe environment for residents and staff
Participate in quality improvement initiatives to enhance patient care and outcomes
Collaborate with the interdisciplinary team to develop and implement individualized care plans
Requirements:
Current LPN licensure in the state of Pennsylvania
Ability to work varied shifts, Days and Weekends
Strong communication and interpersonal skills
Demonstrated ability to work effectively in a fast-paced environment
