Duties
- Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts.
- Monitor and document patient vital signs, including blood pressure, pulse, respiration, and temperature, ensuring accurate medical records in Electronic Health Record (EHR) systems.
- Support medication administration following home healthcare regulations and protocols while maintaining strict patient confidentiality in accordance with HIPAA standards.
- Observe and report changes in patient condition through detailed patient observation and communication with healthcare teams to ensure timely interventions.
- Provide specialized care such as Alzheimer's care, dementia care, memory care, hospice support, and end-of-life assistance with compassion and professionalism.
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe environment for patients.
- Assist with medical documentation and maintain accurate records of patient care activities to support continuity of care.
Qualifications
- Proven experience working as a CNA or HHA in settings such as home health agencies, nursing homes, assisted living facilities or hospice environments.
- Demonstrated knowledge of medical terminology, medical documentation procedures, and home health care regulations.
- Skilled in patient personal care assistance including catheter care and helping with ADLs while respecting patient dignity.
- Ability to perform CPR and First Aid confidently; Basic Life Support (BLS) certification preferred.
- Experience working with geriatric patients or individuals with developmental disabilities or cognitive impairments like Alzheimer’s disease is highly valued.
- Familiarity with EMR/EHR systems for documentation; experience working with clients requiring dementia or memory care is a plus.
- Physical ability to lift heavy loads safely and assist with patient transfers using proper techniques such as Hoyer lifts or other transfer methods.
- Excellent communication skills for client interaction and teamwork; understanding of home & community care best practices is essential.
