Position Summary
We are seeking a compassionate and experienced Field Case Manager to visit clients across various facilities and ensure their overall well-being. This role focuses on assessing patient needs, monitoring their physical and emotional health, and ensuring appropriate services and supports are in place. The ideal candidate is patient-centered, organized, and comfortable working independently while collaborating with interdisciplinary teams.
Key Responsibilities
Conduct regular on-site visits to patients in various facilities (such as nursing homes, assisted living facilities, community)

Assess patients’ physical, emotional, and psychosocial well-being

Ensure patients’ needs are being met, including medical, social, and supportive services

- Identify concerns or changes in condition and communicate findings to appropriate care teams
Coordinate with facility staff, families, and healthcare providers as needed

Maintain accurate and timely documentation in accordance with organizational and regulatory standards

Follow all confidentiality, safety, and compliance guidelines

Qualifications
Current, unrestricted driver's license

Experience working with vulnerable populations, long-term care, behavioral health, or community-based settings preferred

Strong assessment, communication, and interpersonal skills

Ability to work independently and manage a flexible schedule

Reliable transportation and willingness to travel between facilities

Basic computer skills for documentation and reporting
