Requirements
- Current unrestricted Licensed Practical Nurse (LPN) license in the state of practice
- Current BLS Certification
- Valid Driver's License
- Long-term care, home health, or hospice experience preferred but not required
What You'll Do As an LPN, you will provide compassionate, high-quality patient care while working collaboratively with healthcare teams to support positive patient outcomes. Responsibilities include:
- Providing direct patient care and monitoring patient conditions
- Administering medications and treatments as ordered
- Monitoring vital signs and reporting changes in patient status
- Documenting care accurately within electronic health record systems
- Assisting with care planning and patient assessments
- Supporting physicians, RNs, and interdisciplinary care teams
- Providing wound care and other nursing procedures as delegated and appropriate
- Educating patients and families on care plans and treatments
- Promoting a safe, compassionate, and patient-centered environment
- Following all applicable policies, procedures, and clinical standards
