We are seeking Licensed Registered Nurses to fill Home Health positions in Macomb, Wayne, Oakland, Genesee and Livingston counties based on candidates's preference of work area .In this position you will provide case management and clinical care while providing interventions, education and supportive care to clients in a home setting. Extensive medical knowledge as well as a compassionate, positive, hard-working attitude is a must.
Job Responsibilities:
- Assist patient in learning appropriate care plan to reach the patient goals for improvements in health and function.
- Records clinical and progress notes at the time of the visit to ensure continuity of qualitative client care in a timely basis.
- Re-evaluates and records the clients care needs at each home visit
- Continually assess and revise the nursing care plan, and participate with care team in the multidisciplinary care plan, as appropriate.
- Educate and instruct patients, family members, or other patient representatives, in rehabilitative care and activities necessary to promote the client's health, safety and independent living.
Job Requirements:
- A current license as a licensed Registered nurse in Michigan
- A graduate of a nursing program from accredited college
