TREK Healthcare is seeking a compassionate and experienced Part-Time Nurse Practitioner to join our growing team. This position will provide care to palliative care patients throughout the Missoula and Bitterroot Valley areas and offers a flexible schedule of 20–30 hours per week. The role provides a high level of autonomy and the opportunity to develop meaningful relationships with patients and families while making a lasting impact on their quality of life.
The Nurse Practitioner will collaborate closely with physicians, nurses, and interdisciplinary team members to provide high-quality, patient-centered care in home and community settings.

Essential Responsibilities
- Conduct comprehensive patient assessments and physical examinations.
- Evaluate, diagnose, and manage acute and chronic health conditions within scope of practice.
- Collaborate with physicians and interdisciplinary team members to develop and implement individualized care plans.
- Order, perform, and interpret diagnostic tests and laboratory results.
- Prescribe medications and treatments in accordance with Montana law and organizational policies.
- Provide symptom management and palliative care recommendations for hospice patients.
- Participate in care conferences and interdisciplinary team meetings.
- Educate patients, caregivers, and families regarding diagnoses, treatment plans, medications, and disease management.
- Complete accurate and timely clinical documentation in the electronic medical record.
- Ensure compliance with federal, state, and accreditation standards.
Qualifications
Required:
- Current Montana Nurse Practitioner license in good standing.
- Current DEA registration and prescriptive authority.
- Master's or Doctorate degree from an accredited Nurse Practitioner program.
- Current BLS certification.
- Valid driver's license and reliable transportation.
Preferred:
- Previous Home Health, Hospice, Palliative Care, Primary Care, or Geriatric experience.
- Experience working independently in community-based healthcare settings.
- Knowledge of Medicare Conditions of Participation and hospice regulations.
