The Outreach Case Manager will provide outreach, support, and linkage to adults with co-occurring disorders, mental illness and substance use disorders in Morris county. This position requires strong interpersonal skills to make connections with clients in the community, assist them with co-occurring disorders, offer support resources, transportation (in agency provided vehicle), and a path toward wellness.
Timely daily documentation of services provided and quarterly target numbers must be reached for grant fulfillment.
Schedule is 35 weekly hours:
- Mon & Fri 8:30am - 4:30pm
- Tues & Wed 9am - 5pm
- Thu 1pm - 8pm
- Schedule may be flexed to accommodate intermittent weekend hours when necessary to lower client barriers in accessing services.
Requirements
- BA or BS in Social Work, Psychology, or related field required.
- CPRS in lieu of education requirement is acceptable
- Valid NJ Drivers License required
