Advocate for resident rights, preferences, and overall well-being

Communicate with families (with resident consent) regarding care updates and discharge planning

Coordinate referrals and services such as community resources, financial assistance, and supportive programs

Assist residents with financial and legal processes (Social Security, Medicare, Medicaid, VA benefits, etc.)

Build strong relationships with residents and collaborate with staff to support individualized care
-
Participate in interdisciplinary care plan meetings and help develop effective care strategies

Address and help resolve resident or family concerns and grievances
-
Complete social service assessments and discharge planning documentation
-
Maintain accurate, timely records in the electronic medical system

Provide guidance and oversight to social services team members as needed

Experience in a long-term care setting is preferred but not required

Experience with electronic medical records (EMR)

Strong communication, problem-solving, and organizational skills

Ability to manage sensitive situations with professionalism and empathy
