The Role
We are looking for a focused and empathetic Registered Nurse (RN) Case Manager to join our clinical team. In this role, you will be the primary coordinator of care, ensuring every patient receives a holistic and individualized plan that addresses their physical, spiritual, and emotional well-being.
Key Responsibilities
- Clinical Management: Oversee a dedicated caseload of patients within private residences or facility settings.
- Patient Assessment: Perform initial and routine comprehensive evaluations to ensure care remains current.
- Care Planning: Create, execute, and adapt personalized care strategies for each unique family situation.
- Education: Guide patients and their support systems on disease progression, symptom management, and the hospice philosophy.
- Collaboration: Work closely with our interdisciplinary group—including physicians, chaplains, and social workers—to provide seamless care.
- Meetings & Documentation: Participate in mandatory bi-weekly in-office Interdisciplinary Team (IDT) meetings and maintain precise, real-time records in our EMR.
- Coverage: Join the on-call rotation as needed to ensure 24/7 support for our patients.
Candidate Requirements
- Licensure: Active, unrestricted Georgia Registered Nurse (RN) license.
- Experience: At least 1 year of experience in hospice is highly preferred.
- Critical Thinking: Sharp clinical assessment skills and the ability to work independently.
- Communication: Strong interpersonal and organizational skills.
- Mobility: Must have a valid driver’s license and dependable transportation for in-person patient visits.
