Lexington Center, the largest provider of outpatient and residential substance use disorder services in the Hudson Valley, is seeking a Licensed Practical Nurse (LPN) to provide medication administration services within its Comprehensive Outpatient Program (COP) located in Poughkeepsie, NY. The LPN will administer methadone and other approved medications, support nursing assessments, and provide direct patient care under the supervision of a Registered Nurse and/or physician. This role is essential to ensuring safe, compliant, and effective medication-assisted treatment (MAT) for our clients.
Responsibilities:
- Ensure patient care is delivered in accordance with internal policies and external regulations
- Maintain complete and accurate records of client care and medical data
- Dispense methadone per NYS OASAS/DEA regulatory guidelines
- Recognize, treat, and report changes in clients’ physical and emotional conditions related to methadone maintenance
- Maintain adequate supplies and equipment needed for medication dispensing
- Participate in training and in-service education sessions
- Use internal tracking systems to record pertinent data for quality assurance
- Provide direct patient care to methadone clients
- Ensure all medication dispensing fully complies with clinic policies, federal, and state laws/regulations
- Conduct medical assessments of clients and implement preventative measures or recommendations as needed
- Assist clients with arranging physical examinations to support overall health
- Provide oversight and guidance to staff attending to medically involved clients
- Record medical data and observations in client charts routinely
- Witness “observed urine screens” as necessary
- Maintain thorough documentation of medical and psychiatric issues, meetings, case conferences, and client specifics, ensuring follow-up as required by DSS, OASAS, and internal policies
Qualifications:
- Current LPN license in New York required
- Minimum 2 years healthcare/clinical experience; experience in inpatient detox, behavioral health, or substance use treatment preferred.
- Strong documentation, communication, and clinical assessment skills
