Duties
- Conduct comprehensive evaluations of patients’ physical, cognitive, and emotional needs to develop tailored treatment plans.
- Assist patients with Activities of Daily Living (ADLs), including dressing, grooming, feeding, and mobility exercises to promote independence.
- Coordinate discharge planning by preparing patients and families for transition from inpatient or outpatient settings to home or community environments.
- Work with individuals with autism, disabilities, geriatrics, pediatrics, and those in post-acute care or skilled nursing facilities to optimize functional outcomes.
- Implement therapy interventions in various environments such as inpatient units, outpatient clinics, schools, home health visits, and early intervention programs.
- Collaborate with multidisciplinary teams including physicians, speech therapists, case managers, and caregivers to ensure comprehensive patient care.
- Maintain accurate documentation of care plans, progress notes, and treatment outcomes while adhering to medical terminology and physiology standards.
Skills
- Strong knowledge of anatomy and physiology to inform effective therapy strategies.
- Excellent understanding of medical terminology relevant to diverse healthcare settings.
- Experience assisting with ADLs and developing individualized care plans for patients with various conditions.
- Proven ability in discharge planning and case management across inpatient, outpatient, home health, and skilled nursing environments.
- Skilled in working with individuals with autism, disabilities, geriatrics, pediatrics, and those requiring early intervention services.
- Familiarity with ICU experience and acute care settings to manage complex cases effectively.
- Ability to coordinate therapy services within outpatient clinics or school-based programs while maintaining compliance with regulatory standards.
