Immediate Hiring Need!! Part-time Position!!
Under direct supervision of the Program Director and Program Coordinator, the Licensed Vocational Nurse (LVN) provides direct support for individuals with intellectual/developmental disabilities, including supporting participants with Restricted Health Care Needs at our GARDEN Program in Brentwood. This is a part-time position. Must be available to work 6-8 hours on Monday and Tuesdays.
Responsibilities:
- Works with the program's Registered Nurse to develops and monitor Restricted Health Condition Care Plans for participants with a restricted health condition (such as gastronomy tube, colostomy, finger stick testing, or use of oxygen) as part of the participant’s Needs and Services Plan.
- Works with program's registered nurse and Coordinator to develop and conduct 40 hours of staff development training for all GARDEN employees related to restricted health care needs.
- Works with the program's Registered Nurse to trains and certify staff in Restricted Health Condition Care Plan procedures, including medication administration.
- Provides documentation that the participant and his/her authorized representative (if any), the participant’s physician, and the placement agency participated in the development of the plan.
- Performs procedures if the participant needs medical assistance.
- Reviews the plan annually or as required by the physician.
- Attends Intake and ISP meetings as requested by the Program Director.
- May assist in obtaining documentation from the participant’s physician of the stability of the medical condition, medical conditions that require services or procedures, specific services needed, the participant’s ability to perform the procedures, and that the participant does not require 24-hour nursing care.
- Engages in activities and community outings with program participants and staff.
- Notifies the Director/Coordinator of any accidents, special incidents, crises, health and safety issues, potential problems, unusual situations, complaints, challenges, special projects, etc., and writes initial Special Incident/Accident reports as witnessed.
Requirements:
- Valid and current Licensed Vocational Nurse certification from the state of California.
- Two or more years’ experience in human service or educational organizations, at least a portion of which has been with programs for persons with intellectual/developmental disabilities.
- Minimum of one year of varied work experience as a LVN, preferably in either an acute or home health setting
- Knowledge of computers and software, with the ability to troubleshoot basic problems.
- Must possess a valid CA driver’s license, proof of current car insurance and an acceptable DMV printout. Willingness to drive agency vehicles for outings.
- Ability to lift/transfer/push/pull/bend/turn/pivot up to 50lbs. occasionally while assisting individuals including those in wheelchairs as a support in daily program activities and personal care needs.
- LiveScan criminal record clearance (or exemption) prior to start.
- Health Screening and TB Test clearance. Prior to hire.
- Ability to sustain a forward bend occasionally while assisting individuals in assembly tasks, table top activities and eating lunch.
- Follow agency policies and safety procedures on an ongoing basis. Other related duties as assigned.

Great if you have:
- Knowledgeable of Lanterman Act regulations, Regional Center guidelines, and State of California Licensing regulations.
- Certified in First Aid/CPR/AED by a qualified instructor.
