What You’ll Do
- Provide comprehensive physical therapy evaluations and individualized treatment plans.
- Deliver high-quality care and patient education in the homecare setting.
- Communicate effectively with physicians, caregivers, and the rehab team to ensure seamless care.
- Participate in case conferences, admissions, and discharge planning.
- Complete documentation accurately and in compliance with agency standards.
- Assess and recommend adaptive equipment as needed.
- Supervise and instruct health team personnel as appropriate.
What We’re Looking For
- Rhode Island Licensed Physical Therapist (required)
- Strong communication and documentation skills
- Ability to work independently while collaborating with an interdisciplinary team
- Passion for patient-centered, evidence-based care
