RN Case Manager who has Experience in Home Health Private Duty Nursing for patients that are dependent on G-tube, Tracheostomy and Ventilator. RN must be experienced with Pediatrics and Adult patients. We may begin by sending you to do SOC or do a new intake assessment in all areas of Los Angeles and the surrounding cities. At first you may begin with 5 to 10 patient Case Management. Telehealth is provided as well as going to patient's home for recertification. You may be required to teach LVNs on complex procedures such Tracheostomy suctioning and Ventilator circuit monitoring. We are willing to provide training shadowing as well as conducting comprehensive plan of care. customer service skills/compassionate explaining to families of patient or to patient the progress of care and the purpose of home health and LVNs working in the home. You can work from home or in the office or mix. We are willing to Negotiate. The ideal candidate will possess extensive clinical expertise, including critical care, trauma, and hospital medicine, combined with a passion for patient-centered care and case management. In this role, you will coordinate comprehensive care plans, facilitate effective communication among healthcare professionals, and ensure optimal patient outcomes through meticulous assessment and management. Duties

- Conduct thorough patient assessments to determine medical needs, utilizing vital signs, physiological knowledge, and diagnostic evaluation techniques.
- Develop and implement individualized care plans that align with patient needs, hospital policies, and health regulation standards.
- Collaborate with multidisciplinary healthcare teams—including physicians, social workers, and therapists—to coordinate discharge planning and post-acute care services.
- Manage utilization review and medical record documentation within Electronic Health Records (EHR) systems to ensure compliance with HIPAA regulations.
- Facilitate communication with patients and their families to explain treatment options, discharge instructions, and care goals effectively.
- Oversee case management activities in diverse settings such as hospital medicine, ICU, emergency room, home health, hospice care, or clinic environments.
- Monitor patient progress through ongoing evaluation of clinical data and vital signs to adjust care plans accordingly.
- Ensure adherence to health regulation policies while maintaining high standards of patient service and safety.
Skills
- Extensive critical care experience including ICU, emergency medicine, trauma centers (Level I & II), and infectious disease care.
- Proven expertise in hospital experience across medical-surgical units, pediatrics (PICU), geriatrics, behavioral health, and post-acute settings.
- Strong knowledge of medical terminology, physiology, diagnostic procedures, and clinical case management principles.
- Proficiency in EMR/EHR systems for accurate documentation of patient records and medical history management.
- Familiarity with HIPAA compliance standards to safeguard patient information during all stages of care coordination.
- Excellent client communication skills with the ability to collaborate effectively with healthcare professionals across disciplines.
- Experience managing patients in diverse environments such as home health or outpatient clinics; prior managed care or utilization management experience is advantageous.
- Ability to perform comprehensive patient assessments including vital signs measurement and clinical evaluation to inform case management strategies.
