VNA Health Care is seeking a dedicated and compassionate Pediatric Nurse Practitioner (PNP) to join our team of mission-driven healthcare professionals. The Pediatric Nurse Practitioner will play a vital role in delivering comprehensive pediatric primary care services, promoting preventive health, and improving health outcomes for underserved populations.
Essential Responsibilities
- Provide comprehensive pediatric primary care services for infants, children, and adolescents.
- Conduct well-child exams, developmental screenings, and annual preventive visits.
- Diagnose and treat acute and chronic pediatric conditions.
- Order, perform, and interpret diagnostic tests and laboratory studies.
- Prescribe medications and develop evidence-based treatment plans.
- Administer and recommend immunizations according to established guidelines.
- Educate patients and families regarding growth and development, nutrition, disease prevention, and treatment plans.
- Coordinate care with physicians, behavioral health providers, nurses, specialists, and community resources.
- Maintain accurate and timely documentation in the electronic health record.
- Support quality improvement initiatives and population health efforts.
- Promote VNA Health Care's mission of improving the health and well-being of the communities we serve.
QualificationsRequired
- Master's or Doctoral degree from an accredited Pediatric Nurse Practitioner program.
- Current Illinois Advanced Practice Registered Nurse (APRN) license.
- Current Pediatric Nurse Practitioner certification.
- Current Basic Life Support (BLS) certification.
- Strong clinical assessment, communication, and patient education skills.
Preferred
- Bilingual in English and Spanish.
- Experience working in community health centers, FQHCs, or other safety-net healthcare settings.
- Experience caring for diverse and underserved patient populations.
- Familiarity with electronic health records and team-based care models.
Pay: $105,000.00 - $140,000.00 per year
