ESSENTIAL DUTIES AND RESPONSIBILITIES

- Conduct in-home or facility-based visits to hospice patients for initial assessments, face-to face visits, routine follow-up visits, and symptom management.
- Collaborate with the Medical Director or Associate Medical Director and the IDT to develop, implement, and revise individualized care plans.
- Provide expert pain and symptom management consistent with hospice philosophy and guidelines.
- Serve as a clinical resource to hospice nurses and other staff.
- Participate in IDT as needed.
- Educate patients and families on disease progression, end-of-life expectations and hospice treatment options.
- Document all patient care accurately and timely in the electronic medical record.
- Participate in on-call coverage as needed.
- Comply with company policies, procedures, and guidelines.
Location: La Grange, Texas
Shift: PRN- up 12 hours per week

Requirements:
- Minimum 2 years of NP experience
- Master’s or Doctoral degree in Nursing from an accredited program
- Current and unrestricted Nurse Practitioner license in the state of Texas
- Certification as a Nurse Practitioner (AGNP, FNP, or ANP preferred)
- Current DEA Registration and prescriptive authority in good standing;
- Valid Texas Driver’s License, automobile insurance, and reliable transportation are required
