Community Engagement Specialist

Prime Providers is seeking a highly motivated and relationship-driven Community Engagement Specialist to join our growing team based out of our Ontario, California office.
This role is responsible for developing strategic partnerships, increasing community awareness, and driving referral growth throughout the Inland Empire, Riverside County, and San Diego territories. As a key member of our Business Development team, you will serve as the face of Prime Providers within the community while helping families access critical healthcare and support services.

Territory Coverage
This position is based out of our Ontario office and supports growth and community engagement efforts throughout:
- Inland Empire
- Riverside County
- San Diego County
Regular local travel throughout assigned territories is expected.
What You'll Do
Business Development & Community Engagement
- Develop and maintain referral relationships with Regional Centers, hospitals, physicians, discharge planners, case managers, schools, healthcare providers, and community organizations.
- Execute outreach strategies that drive referral growth and increase market presence.
- Represent Prime Providers at community events, resource fairs, conferences, networking events, and professional meetings.
- Identify opportunities to expand services and strengthen community partnerships.
Referral Growth & Relationship Management
- Generate and maintain a robust referral pipeline.
- Serve as the primary liaison for referral sources within assigned territories.
- Track outreach activities, referral trends, and growth opportunities.
- Collaborate with operational and clinical teams to ensure a seamless referral experience.
Community Education & Advocacy
- Conduct presentations, in-services, and educational workshops.
- Promote awareness of Prime Providers' services and programs.
- Advocate for individuals and families seeking healthcare and community-based services.
What We're Looking For
Required Qualifications
- Associate degree in Healthcare Administration, Public Health, Social Work, Business, Marketing, or related field.
- Minimum 3 years of experience in healthcare outreach, community engagement, admissions, business development, or related healthcare settings.
- Proven ability to build and maintain professional relationships.
- Excellent communication and presentation skills.
- Strong organizational and time management abilities.
- Valid California Driver's License and reliable transportation.
Preferred Qualifications
- Bachelor's degree.
- Experience working with Regional Centers, Home Health, Behavioral Health, Nursing, or Community-Based Services.
- Existing relationships within Southern California healthcare and community service networks.
- Bilingual abilities are a plus.

Pay: $70,500.00 - $72,500.00 per year
