Licensed Practical Nurse - All Shifts
Job Summary:

Responsibilities:
Provide direct patient care to residents, including medication administration, wound care, and vital sign monitoring
Develop and implement individualized care plans in collaboration with the interdisciplinary care team
Monitor and report changes in resident condition to healthcare providers and family members
Maintain accurate and up-to-date records of resident care and services
Participate in quality improvement initiatives to improve patient outcomes and satisfaction
Collaborate with other departments, such as rehabilitation therapy and social services, to provide comprehensive care

Requirements:
Current LPN licensure in the state of Ohio
High school diploma or equivalent required
1+ year of experience in a long-term care or rehabilitation setting preferred
