About the Opportunity
We are seeking a compassionate and motivated Part-Time Adult-Gerontology Nurse Practitioner to join our growing team.
This is an ideal opportunity for a clinician who is passionate about geriatrics and values the time to build meaningful relationships with patients while delivering exceptional in-home primary care. You'll practice with a high degree of autonomy while collaborating closely with an experienced interdisciplinary care team to provide comprehensive, evidence-based care.
As an early member of our practice, you'll have the opportunity to help shape an innovative model of care designed around improving the patient experience, preventing unnecessary hospitalizations, and helping older adults age safely at home.
This position begins as a part-time role with a clear opportunity to grow into a full-time position as the practice expands.

Job Responsibilities/Duties
- Provide comprehensive, high-quality primary care in patients' homes and other community-based settings.
- Diagnose and manage acute and chronic medical conditions using evidence-based clinical guidelines.
- Review laboratory and diagnostic results and provide timely follow-up.
- Focus on preventive care, chronic disease management, quality metric performance, and population health initiatives.
- Conduct post-hospital, emergency department, and rehabilitation transition-of-care visits.
- Collaborate with physicians, specialists, care managers, and community resources to coordinate patient care.
- Maintain accurate and thorough documentation within the electronic medical record, including diagnoses, medications, problem lists, and preventive care.
- Participate in ongoing practice improvement initiatives and other responsibilities within the scope of practice.
- Average 4–5 home visits per day during an 8-hour clinical day.
Preferred Qualifications
- Adult-Gerontology Nurse Practitioner certification with active New Jersey licensure. Family Nurse Practitioners with significant adult or geriatric primary care experience are encouraged to apply.
- Minimum of 5 years of Nurse Practitioner experience in primary care.
- Geriatric, home-based primary care, concierge medicine, or home health experience is strongly preferred.
- Current New Jersey prescriptive authority and DEA registration.
- Excellent communication, clinical judgment, and relationship-building skills.
- Comfortable using EMRs and telehealth technology.
