American Traveler is seeking an experienced RN House Supervisor for a 13-week travel assignment at a 180-bed skilled nursing facility in Rochester, NY, requiring prior charge experience and a valid NY RN license.

180-bed skilled nursing facility (SNF) consisting of 5 distinct units
-
Units include a neurobehavioral dementia/Alzheimer's unit with residents exhibiting violent behaviors, a rehab unit (post orthopedic surgery and cardiac procedures), a secure dementia unit for elopement-risk residents, and two long-term care units

Patient population includes residents with diagnoses of dementia, Alzheimer's, DM, CVA, cardiac conditions, and orthopedic post-surgical needs

Average daily census of 180 residents

Specialized equipment in use includes wound vacs, PICC lines, nephrostomy tubes, tube feeds, indwelling catheters, and colostomies

Epic EMR preferred

Schedule consists of three 8-hour shifts and one 12-hour shift per week (36 hours total)

Shift times range between 11p–7a (8 hrs), 3p–11p (8 hrs), and 7p–7a (12 hrs)

Every other weekend coverage required

Position primarily covers evenings, nights, weekends, and holidays

Active NY RN license required
-
Current BLS certification required
-
Minimum of 2 years of RN experience required
-
Prior charge or supervisory nursing experience required
-
SNF experience required
-
Proficiency in code management, de-escalation techniques, and managing assaultive behavior
-
Experience with PICC line care and management, IV insertion and maintenance, and wound vac management
-
Knowledge of regulatory requirements including DPH, TJC, and OSHA
-
Experience with centralized staffing, productivity standards, and staffing requirements
-
Familiarity with federal and state labor laws including FMLA and scope of practice regulations
-
2 professional references from a supervisor within the past 2 years required for consideration
-
Full 7-year work history with all gaps explained required for consideration

Must reside more than 75 miles from the facility to be eligible
-
The House Supervisor serves as a clinical resource and problem-solver across all five units during off-shifts
-
Responsibilities include facilitating patient flow, mentoring and coaching staff, incident reporting, infection control, and sentinel event monitoring
-
Additional duties include managing new admissions, overseeing workplace safety, conducting fitness-for-duty evaluations, and ensuring HIPAA and risk management compliance

Any color scrubs are accepted
