Responsibilities
- Assist licensed physical therapists in developing and implementing individualized care plans for home health patients.
- Perform therapeutic exercises, manual therapy techniques such as trigger point therapy, and other modalities to improve patient mobility and function.
- Conduct comprehensive clinical assessments and accurately document patient progress in accordance with clinical documentation standards.
- Operate medical equipment safely and effectively during therapy sessions, ensuring proper usage and maintenance.
- Manage electronic health records (EHR) to ensure timely, accurate, and complete medical documentation and compliance with healthcare regulations.
- Support discharge planning by evaluating patient progress and communicating updates to the healthcare team for seamless transitions of care.
- Provide care across diverse settings including geriatrics, orthopedics, musculoskeletal and neurological conditions.
Qualifications
- Valid certification as a Physical Therapist Assistant (PTA) with current licensure or registration as required by state law.
- Demonstrated experience in home health environments with a strong focus on patient-centered care - experience preferred not required.
- Knowledge of physiology, anatomy, medical terminology, and clinical assessment documentation standards.
- Proficiency in medical documentation practices including detailed progress notes and treatment reports.
- Familiarity with medical equipment operation relevant to physical therapy interventions.
- Experience working with diverse populations including geriatrics, orthopedics, and those requiring acute or subacute care.
