Position Summary:
Provide accurate and complete assessment and documentation of residents consistent with parameters and definitions established for current MDS format. Assists with resident admissions on the Transitional Care Unit. Responsible for Quality Assurance Assessments and other related functions. Services are provided in accordance with the mission of Guardian Angels.
Job Duties:
- Assess/document resident condition utilizing MDS and supplementary documentation tools.
- Provide recommendations regarding appropriateness or lack thereof of projected Assessment Reference Date.
- Assist with admission process for residents.
- Responsible for Quality Assurance Assessments (QAA).
- Creates QAA agenda and gathers reports.
- Responsible for taking QAA minutes and distribution.
- Reviews QAA follow-up items and ensures the tasks have been completed.
- Monitors Quality Metrics.
- Leads QAPI- runs monthly meetings, brings forth concerns, reviews scorecards, holds individuals accountable for their respective sections and responsibilities.
- Ensures regulatory pieces are adhered to and maintained.
- Partners with Therapy to ensure quality metrics are being met.
- Assists with educational needs as needed.
- Reports back to the Director of Quality and Development for further discussion on education or quality needs and concerns.
- Provides direction, coaching, and mentoring to other MDS Coordinators.
- Participates in the Resource Nurse Rotation.
- To assist in providing a dignified, respectful, resident-centered environment for residents.
- Follows universal precautions, facility infection control guidelines, sanitation and safety standards of practice in all work and activities.
- All duties will be performed in accordance with current standards, guidelines, state and federal regulations.
- Uphold confidential information as required by Guardian Angels policy and procedure and as related to HIPAA.
- Uphold the mission, vision, and values of Guardian Angels of Elk River.
- Performs all other duties as assigned.
Qualifications:
- Current Minnesota Registered Nurse licensure.
- RAAC-CT Certification required.
- Minimum of one year of experience in geriatric nursing.
- Prior experience in Experience in ICD 10 coding and MDS scheduling
- Prior experience with completing Minimum Data Set (MDS); familiarity with prospective payment system.
- CPR certified.
- Ability to complete assignments in a timely manner.
- Prior experience with PCC is highly desired. Experience with Microsoft Office Suite 365 required.
- Excellent verbal and written (read, write) communication skills.
- Self-motivated, ability to prioritize.
- Maintains good attendance and punctuality.
