Certified Risk Adjustment Coder Full-Time | In Person | Perris, CA
Hernandez Health Diabetes and Senior Care Clinics is seeking a full-time Certified Risk Adjustment Coder to support our Medicare Advantage patients. New graduates are welcome, but applicants must have a current CRC or equivalent coding certification.
Responsibilities
- Review charts for accurate ICD-10-CM and HCC coding.
- Complete prospective and retrospective chart reviews.
- Pre-chart Annual Wellness Visits and follow-up visits.
- Identify chronic conditions that need annual recapture.
- Confirm diagnoses are supported by MEAT documentation.
- Identify missing, unclear, or unsupported documentation.
- Provide coding feedback and education to physicians, nurse practitioners, physician associates, and staff.
- Maintain accurate problem lists.
- Complete assigned reviews within required timelines.
- Use eClinicalWorks and Cozeva.
- Work with the Quality Improvement team and CPN Choice Medical Group coders.
- Escalate complex coding questions when needed.
Qualifications
- Current CRC or equivalent certification.
- Knowledge of ICD-10-CM coding.
- Understanding of Medicare Advantage and HCC coding.
- Familiarity with CMS-HCC Model V28 or willingness to learn.
- Strong attention to detail.
- Ability to work independently and communicate professionally.
Experience with HCC coding, Annual Wellness Visits, eClinicalWorks, or Cozeva is preferred but not required.
Technology and AI
AI tools may be used to support pre-charting and documentation review. The coder is responsible for reviewing all recommendations and making accurate, CMS-compliant coding decisions.
Position Details
- Full-time contract position.
- Three-month introductory period.
- Mainly in person at the Perris operational hub.
- Some hybrid days may be available after the coder is fully trained and ready.
- Opportunity for extension based on performance.

Pay: $25.00 - $30.00 per hour
