We are looking to hire talented Pediatric Nurse Practitioners for rewarding roles in pediatric hospice care.
Our hospice and palliative care programs offer a broad range of services in the community or facility-based to help patients and their families ease the burden of managing a life-limiting illness or end of life care.
The Pediatric NP Provider Hospice/Palliative Care has two primary responsibilities.

He/she will provide NP-appropriate service to patients enrolled in hospice and may also assist in staffing pediatric palliative care consultation services that function as a program of Hospice and Palliative Care program

He/she may work in a community, long-term care environment, or hospital

He/she has specialist skills in symptom management, communication including goal setting discussions, and hospice eligibility review, and he/she may work independently or as part of a treatment team

In all settings, he/she will provide in-person visits or telehealth visits, as appropriate. Care will consistently comport with best practices, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation

In the role as hospice NP, he/she will coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care

He/she will also contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed

He/she will also help build a teaching curriculum on pediatric aspects of hospice care that will be of use to our hospice workers who are engaged in pediatric care through cross coverage or for when young adolescents are incorporated into adult hospice teams

In providing both direct care and support to other clinicians, the NP will use an evidence-based approach to management and to the use of materials and policies/procedures

Other activities related to quality improvement projects may also be required, as appropriate
QUALIFICATIONS:
MUST have at least 1 year of Provider / Pediatric Nurse Practitioner experience
PNP must have active New York State Provider license in good standing
Experience in oncology, hospice, palliative care highly desired
