We are seeking a dedicated and compassionate Registered Nurse (RN) to join our team at William Penn Nursing and Rehab, a 121-bed skilled nursing facility located in Lewistown, Pennsylvania, within walking distance to The Geisinger – Lewistown Hospital. As a Registered Nurse (RN) at our facility, you will have the opportunity to make a meaningful impact on the lives of our residents and their families.
About the Role:
As a Registered Nurse (RN), you will provide high-quality, patient-centered care to our residents, including those with complex medical needs. Your duties will include:
Assessing and planning individualized care for each resident
Administering medications and treatments, as prescribed by physicians
Developing and implementing individualized care plans
Collaborating with other healthcare professionals to ensure comprehensive care
Educating residents and their families on health and wellness
Responsibilities:
Provide direct patient care to residents, including but not limited to:
Conducting rounds and assessing resident conditions
Administering medications and treatments
Developing and implementing individualized care plans
Monitoring and reporting resident conditions to physicians and other healthcare professionals
Collaborate with other healthcare professionals to ensure comprehensive care, including but not limited to:
Physician and nurse practitioner consultations
Social worker and rehabilitation therapy consultations
Dietary and activities department consultations
Participate in quality improvement initiatives to improve patient outcomes and satisfaction
Maintain accurate and up-to-date records of patient care
Perform other duties as assigned by the Director of Nursing
