Job Summary
We are seeking a passionate and dedicated Occupational Therapist (OT) to join our dynamic healthcare team. In this role, you will deliver comprehensive occupational therapy services to geriatric populations in our skilled nursing facilities. Your expertise will help geriatric individuals improve their functional independence, enhance their quality of life, and achieve their personal goals. You will develop individualized treatment plans, collaborate with interdisciplinary teams, and ensure the highest standards of patient care through evidence-based practices. Responsibilities
- Conduct thorough occupational therapy assessments to evaluate clients’ physical, cognitive, sensory, and emotional needs
- Develop and implement personalized treatment plans tailored to each client’s goals and medical condition
- Assist clients with Activities of Daily Living (ADLs), including dressing, grooming, feeding, and hygiene to promote independence
- Support discharge planning by preparing clients for transition back home or to other care settings
- Collaborate with speech therapists, physical therapists, physicians and case managers to optimize patient outcomes
- Document clinical notes accurately within electronic health records (EHR) and maintain detailed medical documentation in compliance with regulatory standards
- Provide specialized interventions for geriatrics, individuals with autism spectrum disorders (ASD), neurodevelopmental disorders, or those in post-acute care environments
- Educate patients and families on therapy techniques, adaptive equipment use, and strategies for ongoing support at home or school
- Participate in clinical interprofessional collaboration to ensure comprehensive patient care and evidence-based practice implementation
Skills
- Proven experience working with individuals with disabilities across various settings such as inpatient hospitals, skilled nursing facilities, outpatient clinics or home health services
- Strong knowledge of anatomy, physiology, medical terminology, and treatment plan development
- Skilled in implementing occupational therapy programs for patients recovering from injury or illness in acute care or post-acute environments
- Ability to assist clients with ADLs and functional mobility tasks while promoting independence
- Proficiency in clinical documentation and electronic health records (EHR) management to ensure accurate case management records
- Excellent client communication skills to educate patients and families effectively
- Knowledge of occupational health principles applicable to workplace injury rehabilitation or ergonomic assessments
- Familiarity with speech therapy collaboration when addressing holistic patient needs
- Ability to work independently as well as part of a multidisciplinary team in fast-paced environments such as ICU or skilled nursing facilities

Pay: $40.00 - $53.00 per hour
