Nurse Practitioner – Long-Term Care | DFW Metroplex
Paragon Post Acute Care is hiring a full-time Nurse Practitioner to provide long-term care and skilled nursing facility coverage across the DFW Metroplex. This role is ideal for an NP who enjoys rounding, relationship-building with facility teams, and practicing with autonomy—while being supported by a structured schedule and strong operational backbone.
You'll support our specialty services, including Rehab/Physiatry, Pain Management, and Addiction Medicine, caring for patients in long-term care settings.
What You'll Do
- Round in assigned facilities across the DFW Metroplex, including Arlington, Fort Worth, Dallas, Irving, Bedford, Euless, Grapevine, Cedar Hill, Cleburne, Keller, Lancaster, Midlothian, and Southlake.
- Manage a panel of long-term care patients, rounding monthly per facility, with low-volume SNF consults at select facilities.
- Provide patient evaluation and follow-up visits, including:
- post-acute transitions, functional progress, symptom management
- chronic pain management support (within established protocols)
- addiction medicine support (within established protocols)
- Collaborate with nursing leadership, facility, nursing, and therapy teams.
- Document clearly and efficiently in the designated EMR/workflow.
- Coordinate with Paragon physicians and operations team for clinical alignment and continuity.
- Serve as designated coverage for a primary NP's facilities when that NP is out on PTO, ensuring consistent facility presence and service reliability.
- Provide additional coverage when other NPs are out on PTO, ensuring consistent facility presence and service reliability across your assigned facilities.
- Call rotation with other NPs.
Schedule & Support
- Full-time, weekday rounding
- Schedule is managed for you (routing, facility assignments, visit planning support)
- Strong operations support for coordination, logistics, and continuity
- Panel of approximately 25 LTC/SNF facilities across the DFW Metroplex, with monthly LTC rounding cadence

Requirements
- Active NP license in Texas (or ability to obtain)
- Medicare/Medicaid eligible
- DEA Certification
- National certification (AANP or ANCC)
- Experience in SNF/LTC, geriatrics, Family, and acute care preferred
- Comfortable working independently and moving between multiple facilities
- Strong communication with facility staff and family members
- Valid driver's license with a reliable driving record

Job Type: Full-time
Pay: $130,000.00 - $160,000.00 per year
