Position Summary
The Home Health Aide (HHA) provides personal care and assistance to clients under the supervision of a Registered Nurse. This is a per diem, hourly position supporting individuals who need help with daily living activities, mobility, and companionship. The HHA plays a key role in ensuring clients’ comfort, health, and overall well-being while following each patient’s individualized Plan of Care.
Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility.
- Support safe transfers and positioning, using mobility aids like Hoyer lifts and walkers as needed.
- Monitor and record vital signs (temperature, pulse, respiration, blood pressure) and report changes to the supervising RN.
- Help with meal preparation, feeding, and fluid intake tracking according to patient care plans.
- Perform light housekeeping and laundry to maintain a clean, safe, and supportive home environment.
- Follow infection control and safety protocols while maintaining full compliance with HIPAA and patient confidentiality.
- Provide specialized support for clients with dementia, Alzheimer’s, or developmental disabilities.
- Maintain accurate and timely documentation in electronic health record (EHR/EMR) systems.
- Participate in ongoing in-service training and continuing education (minimum of 12 hours annually).
Qualifications
Required:
- Current California Home Health Aide (HHA) certification
- Minimum 6 months of experience in home health, hospital, or long-term care setting
- Reliable transportation, valid Driver’s License, and auto insurance
- Ability to lift 50–75 lbs and perform tasks requiring mobility, bending, and standing
- Strong communication and observation skills with attention to patient safety
Preferred:
- CNA experience or additional certifications (CPR, hospice, or dementia care)
- Familiarity with home health documentation and EMR/EHR systems
- Experience supporting individuals with chronic conditions or disabilities
Required Credentials (must remain current):
- Certified Home Health Aide License
- CPR and First Aid Certification
- TB Test / Chest X-Ray
- Physical Examination
- Driver’s License & Auto Insurance
Compensation:
Per diem / hourly pay based on experience
(Typical range: $23–$28 per hour)
Work Type:
Per Diem / Hourly — flexible scheduling available
Service Area:
San Diego County — travel required within a 20–30 mile radius for in-home visits
