Job Summary
The Sepsis and Stroke Coordinator will work collaboratively with clinical services to improve the outcomes for the CMS Sepsis Program and the Certification of Acute Stroke Ready Hospital. The incumbent will also collaborate with other Mon Health System and community team members to continuously standardize and improve our programs and outcomes. This position will work collaboratively with all hospital and clinic personnel on shared system and performance improvement projects. This includes ensuring that key stakeholders submit timely audit data, preparing reports for committees, and collaborating with the educators to roll-out key initiatives. In collaboration with the department leads, assist with the development and improvement of sepsis and stroke initiatives. Collaboration includes working with the Medical Staff leaders to provide ongoing education for improvement initiatives. Provide quarterly education to the community on Stroke awareness. In collaboration with the Medical Director, will co-lead the hospital Sepsis and Stroke Meetings. Manage the Get with the Guidelines Stroke Registry as well as The Joint Commission Stroke Database and report the analysis monthly to both the Quality & Patient Safety Council, as well as the Stroke Team. Provide an annual report to the Mon Health Medical Center Quality Committee of the Board of Directors. Respond to Code Sepsis/Stroke to observe team and provide guidance as needed to facilitate protocols; follow up with team afterwards on areas of opportunity, providing addition education as needed
Responsibilities
Planning
Ensure continuous survey readiness for Acute Stroke Ready Hospital program by providing education and opportunities for improvement.
Assessment
Synthesize audits, analyze, and lead improvements in the realm of enhancing sepsis and stroke initiatives.
Oversees and Maintains Guidelines
Oversee the regulatory guidelines for Acute Stroke Ready Hospital and CMS Sepsis regulations. Maintain knowledge of new program regulatory guidelines impacting the care of both sepsis and stroke patients.
Education and Teaching
Participate in community education for stroke initiatives. Participate in the triannual Joint Commission reviews.
Knowledge, Skills & Abilities
Light Work: Frequent lifting and/or carrying of objects weighing up to 10 lbs. with a maximum lifting of 25 lbs.
Must have at least 3-year experience in a clinical health care setting. Proficient in Microsoft Excel, PowerPoint, and Word. Excellent critical thinking skills. Strong ability to build and lead diverse teams. Demonstrate excellent communication skills, both verbal and written. Performance improvement experience is preferred, but not required. Data Analytic experience preferred, but not required.
