Overview
We are seeking a dedicated and compassionate Hospice Weekday On-Call Registered Nurse (RN) to provide essential nursing care and support to patients receiving hospice services during weekday evenings. This role offers an opportunity to make a meaningful difference in the lives of patients and their families by delivering high-quality, patient-centered care in a compassionate and professional manner. The ideal candidate will possess a strong background in acute and post-acute care settings, with experience in diverse clinical environments including critical care, emergency medicine, and geriatrics.
Responsibilities
- Respond promptly to patient needs during on-call hours, providing clinical assessments and interventions as required.
- Conduct comprehensive patient evaluations, including vital signs, symptom management, and medication administration, ensuring comfort and safety.
- Collaborate with interdisciplinary teams to coordinate patient care plans, discharge planning, and symptom control strategies.
- Document all patient interactions accurately within medical records while maintaining strict adherence to HIPAA regulations.
- Manage urgent issues such as acute symptom exacerbations or emergent situations with professionalism and efficiency.
- Provide education and emotional support to patients and their families regarding end-of-life care options and comfort measures.
- Perform diagnostic evaluations and assist with intake assessments for new hospice patients as needed.
- Maintain up-to-date knowledge of medical terminology, physiology, infectious disease protocols, and utilization management practices relevant to hospice care.
Requirements
- Valid RN license with current registration in Texas.
- Experience in ICU, emergency room, hospital medicine, or post-acute care settings such as skilled nursing facilities or outpatient clinics.
- Knowledge of medical-surgical nursing, geriatrics, behavioral health, pediatrics, and inpatient/outpatient care environments.
- Familiarity with medical records systems, discharge planning procedures, diagnostic evaluation techniques, and case management processes.
- Strong understanding of medical terminology, physiology, HIPAA compliance, and patient assessment protocols.
- Previous experience with managed care programs or utilization management is advantageous.
- Excellent communication skills with the ability to handle high-pressure situations effectively while maintaining compassionate patient service.
