Job description
We are looking for a reliable LPN (Licensed Practical Nurse) to work both skilled and non-skilled cases throughout Kanawha and Putnam counties. In addition, the LPN will complete assessments on our private pay clients which will be in accordance to The Joint Commission standards. The LPN will work under the supervision of our Director of Nursing and the Charleston Operations Manager. The ideal candidate will be compassionate, organized and able to work autonomously.
Responsibilities include, but are not limited to :
- Completing bi-annual assessments and develop plans of care for our private pay clients.
- Take and record vital signs when needed.
- Help to teach our new Care Professionals skills in the clients home when necessary so that they can provide the best care possible
- Fill in on shifts assisting clients with ADLs and IADLs when no one is available for our high acuity clients.
- Provide skilled care to clients that contract with us for services which may include medication setup, bowel programs, recording vitals, wound care and other simple procedures.
- Provide emotional and psychological support when needed
- Communicate with Clients family or friends to provide advice, comfort and teaching.
Necessary Skills:
- Ability to undertake a variety of diverse care tasks and work in a multidisciplinary team.
- Understanding of confidentiality obligations and nursing best practices.
- Good knowledge of MS Office and data entry.
- Excellent communication and interpersonal skills.
- Compassionate and the ability to stay calm.
- Successful completion of LPN license program is a must.
- The LPN will also work as a fill-in on high acuity cases when a Care Professional is not available.
- The LPN will be guaranteed 30 hours per week.
- The LPN will be supervised by our DON & Operations Manager
Pay: $24.00 - $27.00 per hour
