General Responsibilities: Is responsible to the Executive Director in the development coordination, and delivery of programmatic staff development services to approximately 30 people with developmental disabilities within a community based facility providing a variety of day and residential services.

Works with: Direct support staff, Agency Supervisors, and All people receiving supports and their families
Specific Duties:
A. Support Services for People Receiving Services

Assist direct support staff in identifying and completing necessary evaluations and assessments in order to develop specific individualized program plans.

To assist staff in writing objective plans and behavior modification plans to meet identified goals.

To act as an advocate for the people receiving support, insuring that their rights are protected.

To assist the Executive Director in monitoring of behavioral intervention programs and strategies to assure appropriate implementation of behavioral programs.

To assist the Director of Quality Enhancement, and the Executive Director in assuring the coordination of people receiving support as identified through the interdisciplinary teams.

If necessary, attended interdisciplinary team meetings to assure the goals and objectives are based on specific needs and/or desires of the person receiving supports.

Coordinate curriculum maintenance and development, program enhancements and assessments.

Teach or train staff to teach specialty classes, i.e. human relations, citizenship, art therapy, music therapy, etc.

Assist in coordinating People First Group, or Aktion Club

Act as Volunteer Coordinator and chair Behavior Intervention Committee and Human Rights Committee.

Complete financial records for participants, i.e. checkbooks, representative payee reports, long term care applications, etc…

Will maintain the CONFIDENTIALITY of any or all information relating to the people receiving supports to include financial, personal, administrative, and programmatic matters

To conduct regular audit of ISP records and implementation processes according to state/federal personal outcome measures to assure that quality services are being planned and maintained.

Utilize SD SMART system to remediate file reviews and to access other reports relevant to Dakota Milestones.

Work closely with the Executive Director and the Director of Quality Enhancement in developing, scheduling, and conducting staff orientation and training.

Assists in assuring accurate individual attendance records and assures that appropriate documentation of programs and related records are maintained.

Assists the Executive Director in long range planning for the agency.

To assist the Executive Director and the Director of Quality Enhancement in the monitoring of service delivery to assure appropriate services to people supported.

Provide direct support services for people supported as assigned.

To perform other duties as assigned by the Executive Director.
Qualifications:
A. Education: A bachelors degree, preferably in education,
human services, or a related field.
B. Experience: Must have a minimum of three years experience in working with
people with developmental disabilities . Prefer experience in
working with outcome performance measures within a community
based facility.
C. Substitute: An equivalent combination of education and experience.
Additional Requirements:
A. Achieve and maintain certification in Cardiopulmonary Resuscitation (CPR) and
Basic First Aid.
B. Maintain a valid South Dakota driver’s license.
C. Attend pertinent inservice and special training sessions.
D. Attend all staff meetings.
E. To schedule hours of work to include some time in the residential facilities.
The Board of Directors of Dakota Milestones shall have the prerogative to hire staff whose qualification may vary from the above.
