Perform home health admissions and start‑of‑care visits for new patients.
Conduct comprehensive patient assessments in the home setting.
Complete and document OASIS assessments accurately and in compliance with regulatory standards.
Utilize Matrix EMR for patient documentation and care planning.
Collaborate with interdisciplinary care teams to develop individualized care plans.
Educate patients and families on treatment plans, medications, and home health services.
Ensure all documentation meets state, federal, and agency compliance requirements.
Coordinate care with physicians, therapists, and other healthcare professionals.
Monitor patient conditions and communicate changes to appropriate clinical staff.
Travel between service areas as needed, primarily in Independence with occasional coverage in Overland Park.
