We are seeking a dedicated and compassionate full-time Social Worker (MSW) to join our home health team in Farmington Hills, Michigan.
This is an opportunity to make a meaningful impact by helping patients and families navigate emotional, social, financial, and environmental challenges while supporting their overall health and well-being.

Assess patients to identify psychosocial, emotional, financial, environmental, and community resource needs

Develop individualized care plans based on patient assessments

Provide patient and family counseling, support, and crisis intervention

Assist with community resource coordination and referrals

Support patients and families with advance directives and long-term care planning resources

Collaborate with physicians, nurses, therapists, and interdisciplinary team members to ensure coordinated care

Participate in case conferences and care planning discussions

Complete timely, accurate documentation in compliance with agency, state, federal, CHAP, and Joint Commission standards

Master’s Degree in Social Work from an accredited program

Current Michigan Social Work license (LMSW or LLMSW preferred)

Minimum 1 year of healthcare or home health experience preferred

Strong assessment and care coordination skills

Excellent communication and interpersonal skills

Ability to work independently and exercise sound clinical judgment

Valid driver’s license, current auto insurance, and reliable transportation
