Role Description:
This is a full-time home health field registered nurse position. The RN provides skilled nursing care in patients’ homes and at our partner facilities. Responsibilities include comprehensive assessments, medication administration and management, wound care, monitoring patients’ overall health status, and responding appropriately to changes in their condition.
The RN will manage a full patient caseload, coordinate and prioritize visits, and develop and update individualized care plans in collaboration with physicians and the interdisciplinary care team. Additional responsibilities include educating patients and caregivers on condition management, safety, and wellness while maintaining a respectful, compassionate, and culturally sensitive care environment.
The RN must complete all visit notes and required documentation accurately, thoroughly, and within established timeframes. Documentation must clearly reflect the care provided and each patient’s current clinical condition and needs. The RN is also responsible for maintaining compliance with agency policies and regulatory standards and promptly communicating changes in patient status to the appropriate clinical and administrative team members.
The ideal candidate is organized, adaptable, and responsive. They must consistently review and acknowledge administrative updates, adjust smoothly to scheduling or operational changes, and remain accountable to the clinical and administrative teams while continuing to provide exceptional patient care.
Common visits may include medically homebound or bedbound patients who require skilled clinical care to recover from surgery, manage acute or chronic conditions, receive wound treatment, or transition safely from a hospital or rehabilitation setting back to their home.

Qualifications and Requirements
We are seeking a dependable and adaptable Registered Nurse interested in a full-time field position in home health. The ideal candidate is organized, responsive, and capable of managing a full patient caseload while consistently delivering exceptional care.
- Minimum of two years of field nursing experience in home health.
- Active California Registered Nurse (RN) license and current Basic Life Support (BLS) certification.
- Ability to independently manage a full caseload, including scheduling, patient visits, follow-ups, and care coordination.
- Strong communication skills and consistent responsiveness to updates, messages, and requests from the administrative and clinical teams.
- Ability to acknowledge important communications promptly and follow through on instructions and required actions.
- Adaptability to workflow, scheduling, policy, and patient-care changes while maintaining professionalism and continuity of care.
- Commitment to delivering compassionate, safe, patient-centered care that reflects each patient’s individual clinical needs.
- Ability to complete visit notes promptly, with thorough, accurate, and clinically relevant documentation.
- Experience conducting comprehensive in-home assessments, developing and updating care plans, and coordinating with interdisciplinary team members.
- Proficiency in wound assessment, treatment, monitoring, and documentation in the home setting.
- Strong time-management, organizational, and critical-thinking skills.
- Ability to work independently in a fast-paced field environment while remaining connected and accountable to the administrative and clinical teams.
- Ability to travel to patients’ homes in and around areas above
- Experience using electronic health record systems is preferred.
Pay: $52.00 - $65.00 per hour
