Job Summary
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our dedicated healthcare team. The ideal candidate will possess extensive experience in various clinical settings, including mental health, and hospital environments. As an LPN, you will play a vital role in delivering high-quality patient care, managing medical documentation, and supporting our clients’ recovery journey through precise assessments and medication administration.
Responsibilities
- Perform comprehensive patient assessments, including vital signs and physical examinations, to inform care plans.
- Administer medications and injections accurately while adhering to safety protocols such as sterile technique and HIPAA confidentiality policies.
- Utilize electronic health record (EHR) systems such as Kipu, Cerner, or Athenahealth for documentation of patient care activities.
- Monitor patients’ condition during acute care situations, including ICU or emergency room scenarios, providing critical support during triage and urgent interventions.
- Collaborate with interdisciplinary teams to develop discharge planning strategies and coordinate post-acute or long-term care services.
- Educate clients on health management topics such as pain management, infection control, nutrition, and medication administration.
- Maintain clinical confidentiality policies while ensuring compliance with long-term care regulations and standards such as NCQA or JCAHO.
Qualifications
- Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.
- Proven experience working in diverse healthcare settings including hospitals, mental health, or skilled nursing facilities.
- Strong knowledge of medical terminology, anatomy, physiology, and ICD coding (ICD-9/10).
- Experience with EMR/EHR systems such as Kipu, Cerner, or Athenahealth for accurate medical documentation.
- Demonstrated proficiency in patient assessment techniques including vital signs monitoring and physical examinations.
- Ability to perform specialized procedures such as IV infusion, catheter care, phlebotomy, and sterile processing techniques.
- Familiarity with geriatric care, dementia or Alzheimer’s care, hospice services, and working with individuals with developmental disabilities is highly desirable.
- Excellent communication skills to effectively collaborate with patients, families, and healthcare teams in a respectful manner.

Pay: $28.00 - $32.00 per hour
